Important Forms
GSSC
"PIP" Form
This form is to be used by new players, guest players, or prospective players
who wish to practice with, tryout for, guest play for, or transfer to or
from a GSSC team.
Medical
Release Form
All GSSC players or GSSC guest players must provide this form, notarized,
to the coach before participating in any games.
Roster
This form is for use by GSSC coaches to verify player rosters. Typically,
the coach must have this roster, signed and validated by the DC, with him
before each game.
